Travelling from the UAE to India soon? There’s a new mandatory health declaration form to complete.
Announced on Thursday June 25, India’s civil aviation authority has launched Air Suvidha 2.0, a new portal for passengers to declare their helath status. The new form is a mandatory requirement for all international travellers arriving into India, and must be completed within the day before you travel.
The launch of Air Suvidha 2.0 comes in response to the ongoing Ebola outbreak.

What travellers need to know
The Air Suvidha Self Declaration Form (SDF) can be found here, and should be completed within 24-hours of your travel plans, with passengers required to complete the form before boarding. The self-declaration form asks for your 21-day travel history, exposure history and related symptoms, if you have any.
All airlines require the health declaration to be completed before boarding any flights to India.
Upon submission, the mandatory self-declaration application is automatically forwarded to the Airport Health Organization (APHO) under the Ministry of Health and Family Welfare, Government of India. Passengers will receive an updated application document in their email inbox.
You don’t need to print out the form, simply show it on your phone upon arrival in India at immigration. This keeps the arrival experience contactless, with no physical forms to fill out on landing.
